How we help
Help in hospital
Our hospital support service makes things easier, especially when family can't. We help communicate with staff, assist with meals, overnight stays, and support people with poor cognition.
Returning home
We work alongside you and the hospital discharge team to create a care plan for your safe transition to the familiar sounds and smells of home.
Supporting your recovery
Our Angel team will coordinate your care managing medications, personal support, nursing needs, and arranging transport to any follow-up appointments. We’re here for everything you need.
Did you know?
The new Support at Home (SAH) program's Restorative Care Pathway provides participants with $6k to $12k funding across 12 weeks.
